Extracardiac coronary steal induced by upper limb hyperemia: a feature of internal mammary artery arteriogenesis

چکیده

Abstract Introduction Function of naturally existing internal mammary artery (IMA)-to-coronary anastomoses has been shown by augmented blood supply to the coronary collateral circulation in response IMA occlusion. Theoretically, this functional connection is invertible and can cause steal, verification whose hypothesis would provide alternate proof mentioned evidence. Method This was a prospective observational study including 40 patients with chronic syndrome occlusion, propensity score matched controls (placebo group) without Primary endpoint inter-group difference function (i.e., flow index, CFI) as obtained at 30, 45 60 seconds following proximal balloon CFI ratio between simultaneous mean occlusive pressure divided aortic both subtracted central venous pressure. To provoke steal phenomenon, upper limb hyperemia induced arm cuff deflation 5-minute supra-systolic inflation ipsilateral sensor-wired release immediately after first measurement. Results Between second CFI-measurement, change CFI@45s minus CFI@30s) absent verum group while there recruitment placebo group: 0.000±0.023 +0.009±0.013, respectively; p=0.032. Conclusion Among artificial induction provokes extracardiac expressed temporarily it normally takes place hyperemia. Funding Acknowledgement Type funding sources: Public Institution(s). Management of Catheter Induced Upper Limb Suppurative Thrombophlebitis in a Patient after Coronary Artery Bypass Graft Surgery: a Case Report

Introduction: One of the important complications of peripheral venous catheters is suppurative thrombophlebitis that would be life threatening without treatment. In this report, we introduce a patient who developed purulent thrombophlebitis due to venous catheter in the upper limb after coronary artery bypass graft surgery. Multiple abscess formation was developed on her upper limb after the th...

Ischemia induced by coronary steal through a patent mammary artery side branch: a role for embolization.

Non-occlusion of the internal mammary artery side branches may cause ischemia due to flow diversion after coronary artery bypass grafting. The authors present the case of a 67-year-old man with recurrent angina after undergoing myocardial revascularization with a left internal mammary artery to left anterior descending bypass. He presented with impaired anterior wall myocardial perfusion in the...

Coronary artery steal syndrome in patient after coronary arteries by-pass surgery and left subclavian artery stenting: is the maintenance of a patent internal mammary artery still necessary?

[email protected] The use of the left internal mammary artery (LIMA) is preferred for the revascularization of a stenosed or occluded left anterior descending artery (LAD). Occlusion or stenosis of proximal part of the left subclavian artery (LSA) could induce reverse blood flow in LIMA called coronary subclavian steal syndrome (CSSS) and should be revascularized before coronary artery bypass ...

Free internal mammary artery for coronary bypass.

An improved technique of free internal mammary artery (IMA) to ascending aorta anastomosis for coronary bypass is described. A small patch vein with a suitable side branch or branches is interposed, connecting the proximal end of a detached IMA to the ascending aorta. A single or double free IMA alone or in combination with an additional saphenous vein graft was performed in 77 patients. A tota...

Beta-adrenoceptor-mediated responsiveness of human internal mammary artery

The internal mammary artery (IMA) is currently the preferred conduit for myocardial revascularization. However, pre-operative vasospasm and a hypoperfusion state during maximal exercise may limit its use as a bypass graft. The mechanism of spasm has not been clearly defined. Since β-adrenoceptor activation plays a major role in vasorelaxation, the present study was carried out to investigate th...
